I've been using a similar build for awhile, but I like S/D instead and I use elemental shielding to keep protection up as much as possible. The damage against everyone except ele/guardian seems pretty solid and you can keep up 9/10 stacks of bleeding at range with the earth auto attack, each doing ~120 damage per tick with a few stacks of might.
Earth scepter 2 also gives you 250 toughness for 30 seconds, which is pretty strong with a toughness/healing stacking build.
I've been experimenting with pulling out two fire elementals to add DPS and blast finishers (from the elite) for might stacks and it's working fairly well against classes without the AoE to kill them quickly. It might not seem like a lot when you watch them attack something, but an extra ~1k damage every 3 seconds adds up over a 12 second fight. Giving up a stun breaker has hurt a few times, but I rarely need more than 1 to win a 1v1 fight anyway.
The main weakness is against builds with poison (-healing) and very high DPS from quickness because I only have ~14k hp to soak up damage and wait out their burst. You can only dodge so much when you're being autoattacked 8 times per second for 2-3k damage a pop.
