The computer I am getting has a Nvidia GeForce GT 620M graphics card. I am wondering if this will be good enough to run gw2 on med-high settings. The screen resolution is lower which might help, 1369x768.
If it isn't enough, are the other GeForce chips interchangeable? Would it even be worth it to upgrade?

You will have to check with your laptop manufacturer to see if an upgrade is possible or not. Notebooks do not have standard parts, sizes and locations so you often are stuck with what your manufacturer has available.
It will probably play GW2 at least at very low settings, though your performance will depend heavily on the CPU also.

I am going to be using an i5-3210M ivy-bridge CPU.
I am also going to be using a 128GB cahcing SSD and 12Gb of 1600MHz of Ram.
Can I squeeze medium/high settings out of this?

No it won't be enough for medium/high, it might run with everything on lowest settings.
Honestly that is a pretty expensive setup (ssd and 12gb ram) being wasted with that terrible gpu.
You'll be better of with an i7, 650M+ laptop that has 4GB ram and no ssd. The price would be roughly the same.
As for upgrading. No I doubt laptops that run with 620m's will have a MXM slot.
