If anyone has a good condition ranger build to share, that would be fantastic.

Try this:

With Carrion equipment.
You can ofc switch some skills, like frost trap <=> spike trap, and quickening zephyr <=> protect me, or lightening reflexes. But the core of this build is to provide combo fields for your team and deal lots of condition damage.
If you really like to, you can take this 20 points form nature magic and put them into marksmanship for piercing arrows, but in most cases axe is enough for AOE attack, its either more damage or more utility.

Yea, I would definitely take Piercing over puting points in Nature for dungeons. I would drop QZ and WS points before I would drop Piercing Arrows, personally. Stacking bleeds, poison, cripple, and stuns with piercing is so huge in dungeons.
Also, take Spike trap over frost trap. You'll get the same snare effect plus the bleeds. Yes, you lose the ice combo field, but ice field is really one of the weaker combo fields anyways.

What armor set and accessories would you use? Crafted only please

I would be very careful about stacking bleeds. Other professions can stack them and you will quickly find yourself in a group or event, where the stack is maintained at 25 and you are losing dps. Because of this, I would not recommend Spike trap over Frost unless you just want a longer lasting snare.
I would also recommend Flame trap for a conditional before any other trap. It has a shortest recharge. Usually only Elementalist bring fire damage and they can not stack fire dots, you will -not- reach a 25 stack limit with Flame trap like you can with bleeds.
To me, creating a Bleed build is a bad idea unless its exclusively for arenas. Poison and fire are much less likely to reach the 25 stack limit.

Cool thanks, then would you also go with Chrysocola Accessories?
I currently have a full set of Rampager's exotic, but have not made my accessories yet. So, I was thinking of the Chrysocola set because of the extra Health along with the extra Condition Damage.
Does that sound like a good idea?

You'll only see this happen on boss fights, in which case you won't be using Spike or Frost anyways, so yes, go spike. Frost is pretty much always inferior to Spike Trap and Muddy Terrain.

*edit* You actually might want to grab something with Toughness since you'll be getting a good bit of vit on your gear. It's good to have a balance of toughness and vit since they both have their advantages and disadvantages.
