Hey. To get elementals on, you have to get Melandru event done and its spawn location uncontested. It last for some time , and once it goes contested again elementals will change again into risen.

u could have dodged alot of that damage if u had been pressing dodge as soon as u stood up. I played alot with scepter so i know that combo can be dodged easily because it takes longer for dragon tooth to land than it does for u to stand up.
