As far as I know there's nothing special about the underwater caverns. They go on for a while, and then just stop.

I'm a little surprised there isn't a title for beating the personal story. I mean come on, the exact dialogue from the personal story is, and I quote,
"For this service, I grant you an ancient honor: you shall carry the title "Champion of Orr" from this day forward."
Is the personal story easy to beat? Kind of, yeah. But so is ressing 1000 NPCs, or amassing 200 gold. All it takes is dedication.

For those of you complaining about a lack of explorer title, doesn't Been There, Done That kind of count for that already?

That explanation actually does make sense from a lore perspective. The greatest heroes of all nations, including Orr, would have had access to the Underworld and Fissure of Woe, and thus at least some of them would have had equipment made with globs of ectoplasm and shards of obsidian. It's important to note that globs of ectoplasm only come from salvaging, there are no natural ecto drops anywhere in the game. That makes sense, since we no longer have access to the Underworld and its endless supply of naturally-forming globs of ectoplasm.

All the rares and exotics you get from Orr are presumably the equipment of long-dead heroes similar to the player character who died in the Cataclysm of Orr. In this case I would assume that the ability to salvage ectos from freshly-crafted rare weapons is just a case of gameplay and story segregation; ectos would become a lot harder to come by if you could only obtain them by salvaging specifically Orrian rares and exotics.

Transmuted things are always account-bound, provided they weren't already soul-bound before. HoM items count as transmuted items. There is only one way to get HoM skins, and that's by earning the required HoM points.

That's the only giant I've ever killed and my count is at 9 giants, so I'm pretty sure it counts. To get credit for a champion kill in group events, though, you have to do a ton of damage on your own. This is the same issue that popped up during the Lost Shores event, when the monthly asked us for karka kills. You could kill as many vet and champion karkas as you wanted with the zerg, but unless you were specced for direct damage you would never hurt them enough for their kills to count for your monthly.
